Verdict: Not Photoshopped
It was taken in the shade with a flash.
There is a shadow of the Dear Leader on the small wall under his jacket and in front of his legs that is consistent with a diffused flash mounted on top of the camera.
Besides any Photoshoper who gave him the nose-picking pose would wind up in a camp.
